Opioid Prescriptions Decline in Canada Following Enactment of Adult-Use Marijuana Legalization

The enactment of marijuana legalization in Canada preceded a “marked decline” in the volume of opioids prescribed to patients enrolled in both public and private health care plans, according to data published in the journal Applied Health Economics and Health Policy.
A team on investigators affiliated with the University of Toronto assessed the volume of opioids prescribed, and the amount of money spent on opioids, in the months immediately preceding and immediately following the legalization of adult-use marijuana sales. Researchers obtained claims data for more than 80 percent of all of the opioids prescribed in Canada during the study period (January 2016 to June 2019).
Consistent with the findings of other ecological studies, researchers determined, “The legalization of cannabis coincided with a marked drop in opioid volumes prescribed in Canada.”
Authors concluded: “The findings of this 42-month time-series analysis revealed a steady and significantly consistent decline in the mean and median MED [morphine equivalent dose] per claim for public payer drug plans. However, when comparing the pre- versus post-legalization time periods, the decline in the mean MED per claim was approximately 5.4 times greater in the period following legalization (22.3 vs. 4.1 mg per claim). In addition, total public payer monthly opioid spending reductions averaged $Can95,000 per month before October 2018 [when adult-use sales were legalized] compared to $Can267,000 per month following the legalization of cannabis. Similar findings were also observed within private drug plans. … The findings of this study add to the growing body of evidence that easier access to cannabis for patients with pain may reduce opioid use and partially offset expenditures for both public and private drug plans.”

If you feel that marijuana is everywhere these days, you aren’t wrong. Over the past year, dispensaries around the country have recorded record sales, which means cities and states are seeing tax revenue boosts in the millions and billions. Many people see the recent uptick of cannabis sales as a result of the success of legalization efforts in different states.
Yet, the truth is that marijuana use has been increasing steadily for over 20 years. What do we actually know about this rise in marijuana use, and how can entrepreneurs and investors leverage this information?
Increasing Usage Rates Are Not the Result of Legalization
Despite the dramatic increase in marijuana usage coinciding with a rapid effort to legalize medical and recreational use across the country, researchers doubt that the increase in use is caused by greater accessibility of legal weed. In fact, the opposite seems to be true.
A study from the Public Health Institute’s Alcohol Research Group published in the journal Addiction states, “policy changes appear to have occurred in response to changing attitudes within states and to have effects on attitudes and behaviors more generally in the U.S.” In other words, populations within individual states have, in their own time, started shifting their opinions on marijuana usage and thus become accepting of more widespread use, which in turn has led to efforts for legalization.
However, this increase in use is not true for all age groups. Despite fears that legalization would make marijuana use more attractive to adolescents — for whom cannabis can be dangerous, as it interrupts and fundamentally alters brain development — a federal survey found that marijuana use by teens is currently at its lowest since 1994. What’s more, teenage cannabis consumption has been steadily decreasing since the 1990s, perhaps thanks to greater efforts to educate children about the hazards of using substances too early.
Though legalization is likely to add greater legitimacy to marijuana use, the truth is that most states already have a foundational acceptance of the practice for years before medical and recreational programs are developed and ratified.
People Use Marijuana When Stressed, Lonely, Bored
Though generally marijuana use has seen an upward trend over the most recent decades, there has been a marked uptick in use since the global outbreak of COVID-19 at the start of 2020. The United Nations reports that there has been an increase in the worldwide demand for cannabis — which, in places where legal recreational and medical marijuana programs are not available, has driven black-market cannabis sales to new heights. Fortunately, legal marijuana dispensaries in Chicago, Los Angeles, Portland and other cities have also reported record sales for the year, indicating that when given legal options, users do avoid participating in drug crime.
Though more research is warranted to understand this phenomenon, most scientists believe that the long periods of isolation and social distancing mandates have contributed to the recent increase in drug use. Marijuana’s effects of euphoria and relaxation are brief palliatives for symptoms of anxiety and depression, which have unfortunately increased as a result of pandemic-related fears and government mandates. Yet, it is important to note that most mental health professionals do not advocate for self-medicating with cannabis products, which can exacerbate mental illnesses in the short and long-term. Instead, those suffering from feelings of anxiety or depression should seek professional help and only increase their cannabis use under the direction or supervision of a trusted doctor.

Conclusion
Through the millennia, humans have used cannabis in almost every capacity. Not only is the plant a practical resource for fiber and food, but its psychoactive effects have found dozens of applications throughout human life. Some African and Caribbean religions maintain marijuana use as a spiritual rite, and increasing research has shown that medical applications of the drug can be beneficial in managing several health conditions.
The recent increase in marijuana use is not tied directly to the legalization of the drug; rather, the legalization of cannabis is a reflection of changing social attitudes — about marijuana but also about individual will and the government responsibility. Though it is important to continue tracking use patterns to understand who is using cannabis and why, for now, it seems that marijuana use is a natural development in human history.

New Mexico Gov. Michelle Lujan Grisham (D) on Tuesday reiterated her commitment to legalizing marijuana in the state in 2021.
During a State of the State address, the governor again discussed cannabis reform as a means to generate needed tax revenue amid the coronavirus pandemic and create jobs.
“A crisis like the one we’ve experienced last year can be viewed as a loss or as an invitation to rethink the status quo—to be ambitious and creative and bold,” she said. “That kind of thinking includes, of course, recreational cannabis and the tens of thousands of jobs and hundreds of millions in new revenue it will bring to our state.”

“I have no interest in another year of thousands of New Mexicans eager to get to work and make their future in this industry being told ‘no,’ just because that’s easier than doing the hard work to get to ‘yes,'” the governor said. “When we emerge from this pandemic, we can have the same old economy, with the same old boom-and-bust future, or we can roar back to life, breaking new ground and fearlessly investing in ourselves, in the limitless potential of New Mexicans. I know which future I prefer, and we can begin building it this year.”
Lujan Grisham also included legalization as part of her 2021 legislative agenda that she released earlier this month. She said in a recent interview that she’s “still really optimistic about cannabis” this session.
That optimism is bolstered by the fact that several anti-legalization Democrats, including the Senate president pro tem and the Finance Committee chair, were ousted by progressive primary challengers last year.
Additional pressure to end cannabis prohibition this year is coming from neighboring Arizona, where voters approved legalization in November and where sales officially launched last week.
New Mexico shares another border with Colorado, one of the first states to legalize for adult use. Cannabis is also expected to be legalized across the southern border in Mexico, with lawmakers facing a Supreme Court mandate to end prohibition by April 2021.
A bill to legalize cannabis for adult use passed one Senate committee last year only to be rejected in another before the end of the 30-day session. Earlier, in 2019, the House approved a legalization bill that included provisions to put marijuana sales mostly in state-run stores, but it later died in the Senate.
At least five pieces of marijuana legalization legislation are being prepared in the legislature this year, according to a top lawmaker, and so what the program might ultimately look like is an open question.
Senate Majority Leader Peter Wirth (D) said last week that he’s been having conversations with lawmakers about what needs to be prioritized in reform legislation. That includes ensuring that it promotes social equity and protects the state’s existing medical cannabis system.
Rep. Javier Martinez (D), who has consistently sponsored cannabis reform bills in past sessions, said recently that the “biggest change you’ll see in this bill, which is one of the main points of contention last year, was the creation of a number of different funds, earmarks, tax coming in from cannabis.”
In any case, there’s economic urgency to pass and implement a legal cannabis program. And while no bills have been introduced so far this session, lawmakers expect several to be released as early as this week.
Polling indicates that voters are ready for the policy change. A survey released in October found that a strong majority of New Mexico residents are in favor of legalization with social equity provisions in place, and about half support decriminalizing drug possession more broadly.
In May, the governor signaled that she was considering actively campaigning against lawmakers who blocked her legalization bill in 2020. She also said that she’s open to letting voters decide on the policy change via a ballot referendum if lawmakers can’t send a legalization bill to her desk.
Lujan Grisham isn’t the only governor who’s made a point to highlight marijuana policy in their annual address.
The governor of Nevada stressed during his State of the State speech last week that his budget proposal contains provisions to keep marijuana tax revenue flowing to schools in the state.
In his address this month, New Jersey Gov. Phil Murphy (D) reaffirmed his commitment to passing legislation to get a legal marijuana market up and running after voters approved a legalization referendum in November.
Also, New York Gov. Andrew Cuomo (D) discussed the need to legalize marijuana in 2021 during his State of the State address.
In Connecticut, Gov. Ned Lamont (D) similarly talked about his intent to work with lawmakers to enact legalization during his State of the State address.
Kentucky Gov. Andy Beshear (D) said in his annual address that “it is time to legalize medical marijuana.”
In Virginia, Gov. Ralph Northam (D) said during his State of the Commonwealth speech that cannabis criminalization was intentionally set up “generations ago” to discriminate against people of color, and he called for legalization.

Atlanta: Mayor Takes Executive Action Abolishing Pre-Employment Drug Screens for Many Public Employees

Mayor Keisha Lance Bottoms has issued an executive order suspending pre-employment drug screenings for public employees in non-safety sensitive positions. The citywide order is similar to legislative changes enacted recently in several other municipalities – including New York City and Washington, DC – that have also eliminated drug screening for marijuana and other substances as a condition of employment.
The Mayor categorized the drug testing requirements as “outdated and costly barriers to onboarding new talent in the city of Atlanta.”
Under the new order, “Prospective employees who are not applying for employment in safety sensitive positions shall not be required to undergo a post-employment offer physical examination/drug test.”
Commenting on the executive order, NORML’s Deputy Director Paul Armentano said: “The abolishment of this discriminatory policy is long overdue. The use of cannabis during one’s off hours poses no legitimate workplace safety threat and the tens of millions of Americans who engage in this behavior should no longer be stigmatized or denied employment because of it.”
Because THC’s primary metabolite, carboxy-THC, is lipid soluble, residual levels of the compound may persist in urine for weeks or even months post-abstinence. According to the US Department of Justice, a positive urine test screen for drug metabolites “does not indicate abuse or addiction, recency, frequency, or amount of use; or impairment.”
Data published last year in the scientific journal Occupational Medicine reported that those with a history of cannabis use over the past year are no more likely than non-users to experience an injury at work, regardless of occupation. The study’s conclusion was consistent with those of other studies — such as those here, here, here, and here — finding that adults who consume cannabis in their off-hours are no more likely to suffer workplace injuries than are those employees who abstain from the substance.
